This report should be used to provide information to the International Organization regarding concerns of individual member/chapter behaviors or policy violations. Alpha Delta Pi strives to keep the reporter’s identity confidential, whenever possible. A report may be completed anonymously by omitting the name and contact information from the form. Reporters should be aware, however, that this may limit Executive Office staff and/or International Officers from completing a thorough inquiry/investigation into the matter.
